His side b does much much more knockback
Down b does peachs down smash damage
Up b is a kill move.
Down smash comes out much faster.
some of dr mario's attacks have electricity effect like his forward smash
neutral b has ridiculous enlarged hitbox and last longer
Dair does more damage
double jump height increased
Forward smash always sweetspots
uair has a disjointed hitbox

I think his pills need to have longer hitstun and maybe last a bit longer (maybe even do more shield damage), but no other change besides the universal land cancelling.
As far as the big picture is concerned, its pretty important to make sure that Mario and Doctor Mario are as different as possible, and as such the process over ludicrously overbuffing them needs to go into seperate directions.
The specifics are really fuzzy though, its something im gonna have to think about more before i post my thoughts on it.
On the topic of making clones unique, the video posted showing off Link's insane speed buffs makes me worry about how Young Link may turn out.

The very top of his Up-B is a sweetspot like Luigi's in reverse, but it only hits if none of the previous hits connected. It also goes higher, like Mario's Smash 64 Up-B.
Down-B shoots Pills on both sides, but has a bit more endlag.
Side-B sheet is even longer.
Neutral air stays out for longer and is ridiculously powerful if you hit at the last couple of frames.

Yo Doc main here. "Forward smash always sweetpots" : it's already the case in classic Melee, Doc's Fsmash has less range than Mario's, but does the exact same amount of damage and knockback on all of the hitbox
As Fedora said, we need Doc and Mario to be radically different. As such, I suggest Doc to be more zoning/gimp based, and Mario to be speed/pressing based. So for the Doc :
- more Pill control, like choosing between different angles, speed, etc. - Increased Uair range - SideB puts opponents in helpless state - Dair is Luigi's Dair
